Procedure for Reporting Copyright Infringement

5. A good faith statement that you believe the use of the material in question is not authorized by the copyright owner, its agent, or the law.

6. A statement of accuracy, under penalty of perjury, confirming that the information in your notice is accurate and that you are authorized to act on behalf of the copyright owner.
